11. Hospital Palliative Care Team Patient Leaflet

This pathway is intended for use by Acute ward staff to facilitate an effective discharge for patients approaching end of life.

The pathway is for use with patients who have approximately less than 6 weeks to live. 

For patients with life-limiting conditions who are expected to live longer than 6 weeks please follow the WAHT Discharge/ transfer Policy.
